Paperwork for Appointments

To facilitate a comprehensive consultation please complete your new patient paperwork (PLEASE SEE BELOW) and bring it with you (please download the forms below). To view them, you can download the free Adobe reader from here. Please also bring the following:
  • A list of all of your current medications
  • Any necessary reports, X-rays, or CDs. Please bring CDs for any CT scans done outside the Hoag System
  • Insurance authorization (if required by your insurance)
  • Your current insurance card

Please call the office in advance to ensure that your medical records and tests sent from other doctors or facilities have been forwarded to us.

Appointment & Scheduling

We strive to make scheduling convenient for all our patients:
  • Request an Appointment – Call us or use our online request form.
  • Cancellations & Rescheduling – Notify us at least 24 hours in advance for changes.
  • Telemedicine Services – Virtual consultations are available for select conditions.
